Change your name
We'll need to see evidence of your changed name. Send us a readable copy of your proof of name and include your Blue Badge reference number.
Your proof of name can be a copy of a:
- valid Passport
- valid Driving Licence with photocard
- Certificate of British Nationality
- HM Forces ID Card
- Birth or Adoption Certificate
- Marriage or Civil Partnership or Divorce or Dissolution Certificate
- National Identity card for foreign nationals
Change your address
Let us know your address has changed so that we can update our records.
When you contact us, please include your:
- name
- email address
- old and new home address
- telephone number
Depending on your age, we will need the following documents as proof of address.
If you are under 18, we accept the following to prove your address:
- a letter from your school
- National Insurance letter
If you are over 18, we accept a:
- copy of a Benefits letter dated within the last 3 months
- Council Tax letter dated within the last 3 months
A Blue Badge is issued to you as an individual so it won't need to be amended, replaced or cancelled if you change address.
Change your vehicle
A Blue Badge is issued to you as an individual, you don't need to tell us if you change your vehicle.
